Survey of Activities to Monitor the NYS Plan to Eliminate HCV

The New York State (NYS) Hepatitis C (HCV) Elimination Plan outlines recommendations that will lead towards eliminating HCV as a public health problem in the state. This site survey will collect information on activities and outcomes related to each recommendation to track progress towards implementing the Plan. A summary report will be shared annually and disseminated to the public via the NYS Department of Health Website. For additional information contact hepatabc@health.ny.gov.
